Following guidance from the Health & Social Care Board and a review of our practice policy we will be making changes to prescriptions collections.

From 25th June 2018 there will be 3 options available for collecting prescriptions:

  •     Nominate a Pharmacy

You may nominate ONE pharmacy to collect ALL your prescriptions.

Your prescriptions will only be collected by your nominated chemist. When your prescription has been issued, and the pharmacy has collected it, you will be able to pick up your medication directly from them without calling at the practice. You must ask your pharmacy to collect your prescriptions for you each time you order.


  •   Collect in Person

  •   Nominate a Representative

You may nominate ONE person (16+ yrs) to collect ANY prescriptions on your behalf. They may be asked for ID to confirm their identity.

If you want to provide authorisation for a nominated pharmacy or representative to collect your prescriptions, please use the attached consent form. Pharmacy Collection Consent Form

Forms should be brought to practice reception where your signature will be witnessed. You may be asked to provide photographic ID.
